Upon acceptance of this registration application, I consent that the player(s) and team staff named on this application are able to participate in the Rep B tournament being organized by the Cornwall Minor Hockey Association (CMHA).  I agree to abide by all the regulations, rules, bylaws, policies and Code of Conduct of the Cornwall Minor Hockey Association (CMHA).  I agree to release the Cornwall Minor Hockey Association (CMHA) from all claims for damages from any accidents or injuries which are caused by or arise from participation during any game, practice or function of the tournament.

Privacy Policy

CMHA is committed to respecting and protecting the privacy of our Members, their Associations, individual members, their families and our employees. Players’ and volunteers’ personal information, collected in a database other than the Hockey Canada registry, for the purpose of league and association tournaments will remain with CMHA and no personal info will be provided outside of our use.

Refund Policy

Note :  A minimum $300 admin fee will be charged for any refund requests before Jan 9th, 2026 (11:59 PM).  No refunds will be provided as of Jan 10th, 2026 (12:00 AM).

Offline Payment Instructions

U11 Rep B Tournament Entry Fee is $1450.00.  Teams will only be considered accepted by the tournament convenor once fees have been received in full.

All teams must be ready to play by Friday Jan 30th, 2026 at 8 AM.

Tournament Fees must be paid by etransfer only :
An etransfer for the full amount to - treasurer@cornwallminorhockey.ca    -   please ensure you enter your team info and age category in the notes section when processing etransfer payment, so we know which team to apply the funds to.

Please send a confirmation email once you have sent funds to confirm your spot.  If funds are not received within 14 days, the spot may be opened to another team in line.
